Here is KB4569745 changelog –
Microsoft security experts have worked in this patch to solve a vulnerability during elevation of privilege that inappropriately grants access to cached files when .NET Framework or ASP.NET web apps running on IIS. An invader could gain access to restricted files because he exploited this vulnerability. He would need to send a manually crafted request to an affected server. KB4569745 addresses this by changing how ASP.NET and .NET Framework handle requests.

He would need to be able to upload a specifically crafted file to a web app. KB4569745 patch addresses the vulnerability by correcting how .NET Framework processes input.
